Leaving school is a significant milestone. For many young people and their families, the next challenge is finding a meaningful pathway that provides continued learning, skill development, social connection and support towards greater independence. The Program has been designed to bridge the gap between school and adulthood, providing a structured pathway that helps young people build the skills, confidence and experience needed for their next stage of life. 

Unlike traditional day programs or employment-only services, the Program combines practical life skills, community participation, employment readiness and accredited learning opportunities within one integrated program. 

Skills for Life. Skills for Work. Skills for Independence.

What is the Program?

The Program is a structured post-school pathway that supports young adults with disabilities to continue learning, develop independence and prepare for future opportunities. 

The program provides a balance of practical learning, community participation and skill development, helping participants build confidence, strengthen social skills and explore pathways that support their future goals. Whether that future includes employment, further education, volunteering or greater independence, the Program provides the foundation to help young people take the next step. 

Preparing for What's Next

The transition from school is about much more than finding somewhere to go each day. It's about building the confidence, skills and independence needed for adult life. 

Through the Program, participants continue learning in a way that is practical, purposeful and relevant to their future. They develop communication and relationship-building skills, learn how to navigate real-world situations, build confidence in community settings and gain exposure to workplace environments and expectations. 

Whether a participant's goal is further education, employment, volunteering or increased independence, the Program provides opportunities to develop the skills, experience and confidence needed to take the next step. Through community-based learning, accredited training opportunities and practical skill development, participants are supported to create a pathway that reflects their individual strengths, interests and aspirations. 

Learning Through Real Experiences

Some of the most valuable learning happens outside the classroom. 

Participants regularly engage in community-based learning experiences including cafés, libraries, community events, volunteer opportunities, shopping centers, workplace visits and recreational activities. These experiences allow young people to practice new skills, develop confidence and apply their learning in real-world environments. 

By participating in everyday community settings, participants gain practical experience while developing the confidence and independence needed for adult life. 

Accredited Learning Opportunities

One of the unique features of the Program is access to nationally recognised training opportunities through SGSCC's Registered Training Organisation (RTO). 

Eligible participants can undertake accredited learning that supports future education and employment pathways while developing practical workplace and employability skills. Areas of study may include workplace communication, teamwork, problem-solving and employment preparation. 

This combination of practical learning and accredited training creates meaningful opportunities for participants to continue their education while preparing for future employment and independence. 

Small Groups, Personalised Support

The Program is delivered in small groups with a ratio of 1 staff member to 4 participants, ensuring participants receive personalised support while still being encouraged to develop independence and personal responsibility. 

The small group approach enables stronger relationships, greater engagement and tailored support suited to each participant's goals and aspirations. 

Real Pathways. Real Outcomes.

Every young person has a different vision for their future. 

Some participants are exploring employment opportunities and developing workplace skills. Others are building confidence, strengthening social connections, pursuing further learning or developing the practical skills needed for greater independence. 

The Program provides a supportive environment where young people can continue to learn, challenge themselves and discover what's possible. By focusing on individual strengths and creating opportunities for growth, the program helps participants build a genuine pathway from school into adult life.
